Part I: HIPAA Notice of Privacy Practices
Our Commitment to Your Privacy
Dr. Quinton Pooler, DDS (“we,” “us,” “our,” or “the Practice”) is dedicated to maintaining the privacy of your protected health information (“PHI”). In the course of providing dental treatment, we create records regarding you and the care and services we provide. This section applies to all such records, whether made by Practice personnel or your dentist.
We are required by law to:
- Maintain the privacy of your PHI;
- Provide you with this Notice of our legal duties and privacy practices regarding your PHI;
- Abide by the terms of the Notice currently in effect; and
- Notify you if we are unable to agree to a requested restriction, and notify affected individuals following a breach of unsecured PHI.
How We May Use and Disclose Your Health Information
- Treatment — to provide, coordinate, or manage your dental care, including consultations with other dentists, physicians, specialists, or labs involved in your treatment.
- Payment — to obtain payment for services, including verifying insurance coverage, submitting claims, and billing you directly.
- Health Care Operations — for activities such as quality assessment, staff training, licensing, and practice management.
- Appointment Reminders — we may contact you by phone, text, mail, or email to remind you of appointments or share information about treatment alternatives or related health services.
- Business Associates — we may share PHI with vendors performing services on our behalf (e.g., billing or IT support) under agreements requiring them to protect your information.
- As Required by Law — we will disclose PHI when required by federal, state, or local law.
- Public Health and Safety — to prevent or control disease, report abuse or neglect, or comply with FDA or workplace safety requirements.
- Health Oversight, Legal, and Law Enforcement — for audits, investigations, court orders, subpoenas, or other legal proceedings, and to law enforcement in limited, legally permitted circumstances.
- Other Permitted Disclosures — including to coroners or medical examiners, for organ/tissue donation, for approved research, and for workers’ compensation claims.
Uses and Disclosures Requiring Your Written Authorization
Other uses and disclosures not described above — including most marketing uses of your PHI and any sale of PHI — require your written authorization, which you may revoke at any time, except to the extent we have already relied on it.
Your Rights Regarding Your Health Information
- Inspect and copy your dental and billing records, with limited exceptions.
- Request amendment of PHI you believe is incorrect or incomplete.
- Request an accounting of disclosures made outside of treatment, payment, and operations.
- Request restrictions on certain uses/disclosures, including to family members involved in your care. We are not required to agree, except where you paid out-of-pocket in full and ask that we not disclose that service to your health plan.
- Request confidential communications, such as being contacted only at a specific number or address.
- Obtain a paper copy of this Notice upon request.
- File a complaint if you believe your privacy rights were violated (see below), without fear of retaliation.

Note: Florida law requires practitioner complaints to be submitted in writing/signed — they cannot be filed by phone.
There will be no retaliation for filing a complaint with us or with either agency above.
Part II: Website Privacy Policy
This Website Privacy Policy explains how we collect, use, and protect information gathered through our website at https://drquintonpoolerdds.com/ (the “Website”). It applies only to information collected through the Website — not to information collected by third-party sites we may link to, or by service providers who maintain their own separate privacy policies.
